Dear Sir/Madam,
I hope this correspondence finds
you in good health and high spirits. I am writing to formally request a
correction in the expiry date of my Permanent Medical Registration, as entered
in the online application form submitted for the FCPS-1 examination with form
No. (------------) on 21-06-2023.
Regrettably, it has come to my
attention that the expiry date for my medical certificate of Permanent Medical
Registration was entered incorrectly during the application process. The
correct expiry date for the medical certificate issued by the PMDC (Pakistan
Medical and Dental Council) is 07-05-2028, as evidenced by the attached copy of
the certificate.
Therefore, I humbly request your
kind assistance in rectifying the currently entered expiry date of the
certificate, which currently reads as 17-01-2023, to reflect the accurate
expiry date of 07-05-2028.
